Zakawetee Hiss as Index of Neurosis.

ABSTRACT- ABSTRACT BACKGROUND Literal meaning of Zakawetee Hiss is hypersensitivity of sexual organs. It is believed that Zakawetee Hiss is the cause of dhat, premature ejaculation (PME) and hyper sexuality and is caused by inordinate sexuality of all sorts. The concept of Zakawetee Hiss was thought to be a realm of hakims, but the associated neurotic features have diverted attention towards the psychological aspects of the illness. AIMS To examine the concept of Zakawetee Hiss as formulated by the sexologists of Indo-Pak Subcontinent. To probe parallel concepts in the western sexology. To explore psychiatric comorbidities and to suggest appropriate psychopharmacological mode of treatment. METHOD Several websites were searched for the words like culture bound syndrome, depression, anxiety, somatoform disorders, PME, serotonin, noradrenaline and hypersexuality. It was followed by manual search for research papers, books and articles from psychiatry, urology, sexology and hikmat. Semi- structured interviews were conducted with different hakims. RESULTS Results revealed that Zakawetee Hiss accounts for phenomenon like dhat, PME, hyper sexuality and timidity. Anxiety, depression, OCD and somatoform disorder came out as psychiatric comorbidities. Efficiency of SSRIs has been considered. CONCLUSIONS It was concluded that Zakawetee Hiss is a concept relevant in western sexology. Neurosis is substrate of anxiety, depression, somatoform disorder and psychosexual disorders. Psychological and sexual behaviors in neuroticism have common bases in norepinephrine dysfunction. Looped interaction of CRF, ACTH, norepinephrine and corticosteroid explains genesis of anxiety and depression. Zakawetee Hiss came out to be an index of neuroticism. CLINICAL IMPLICATIONS Zakawetee hiss can be included in serotonergic disorders like depression, anxiety, phobias, OCD, migraine, premature ejaculation, drug dependence, loss of impulse control, irritable bowel syndrome, chronic pains, sleep disorders, eating disorders, cognitive dysfunctions etc. It represents sensory, motor and behavioral dimensions of serotonergic dysfunction. SSRIs as a class of drug may prove its superiority as a treating agent. Theoretically SSRIs would be effective for all dimensions of Zakawetee Hiss.
