ABSTRACT- The case that will be introduced it concerns the treatment, with cognitive-behavioral techniques (deculpablizzation, delay of the answer for the cleaning rituals, assertive and relaxation training, systematic desensitization in imagination), of a fifty-year-old man suffering from a serious paraphilic disease NAS (not otherwise specified) treated Psychotherapist) in a private care-centre of Rome. His paraphilic imaginations concerned fat women (adipophlias), elderly (gerontophilias) and with physical impairments (apotemnophilias). Besides, he introduced an aversion for the female genital organ that had produced a sexual partialism, (fellatio and masturbation) with consequent not consummation of the marriage. The man in matter exclusively submitted him to a psychotherapic treatment to avoid a separation. His wife didn't know of the real causes of the sexual difficulties of her partner that persisted from over fifteen years.
