Studies for Ejaculation Latency Time.

ABSTRACT- Introduction .-Ejaculation is a neurological reflex that involves two mechanisms:the first one or afferent reflex begins by stimulating penis receptors(located in glands and penis shaft),then those stimulus travel through dorsal to pudendal nerves,to sacral cord and finally to the hypothalamus. The second efferent reflex is a response from the hypothalamus centers to parasympathetic nerves and ganglia. Those actions result in release of neurotransmiters,which depolarize perineal muscles,deferents,seminal vesicles,prostate and urethra muscles,and begin rhythmic contractions and seminal emission. Material and Methods.-There are three states of sensitivity and response to sexual stimulation: normal,hyposensitivity and hypersensitivity. To know the lengh of time of the whole reflex arc or partial afferent or efferent reflex time, we have used several tests to measure these parameters and to attempt to find the ejaculation latency time,which is required for PE. treatment:a.-vibratory threshold to ejaculation test,is a very easy test,that consists to put penis over a vibratory machine and wait maximun 4 minutes.We consider normal if the patient can get a rigid erection but not an ejaculation.But if the patient ejaculates less than one minute is equal to a high hypersensitivity. b.-For skin sensitivity test we use an electrode,which is applied to several skin places (we have a map).The machine emit progressive signals to be recognized by patient. For neurological tests(bulbocavernous reflex,evoked genitocerebral potentials,bulbocavernous latency time, bulbocavernous evoked potentials,dorsal nerves latency time.sural and tibial nerves latency time, this last one as a check test) waves amplitude and morphology are so important as conduction times.Lateny time is more important than evoked potencials because indicates pathways involved in the reflex and considers the ability of circuit to transmit signals. Evoked genitocerebral potential reflect the ability for coommunication between brain and genital area Conclusions.-Although variety of factors can interfere with electromyographic registration,these procedures are the best ones for neuroconduction study,normal,hypo and hypersensitivity. We can predict ejaculation latency time and therefore PE. treatment.
